3 PAYMENT PLAN "40/30/30"
1st payment - 40% of total annual policy premium ... due prior to the effective date of the policy.
2nd payment - 30% of total annual policy premium plus $4.00 service charge ... due 3 months from policy effective date.
3rd payment - 30% of total annual policy premium plus $4.00 service charge ... due 6 months from policy effective date.
9 PAYMENT PLAN "20/10"
(available only when that total annual policy premium is $300.00 or more)1st payment - 20% of total annual policy premium... due prior to effective date of the policy.
2nd through 9th payment - 10% of total annual premium plus $4.00 service charge ... due 1st through 8th month after policy effective date.
